The form is a new concept which combines the old style 'Check Request' with the former Membership Approval form, Appendix A, of the local policy. The new form prompts users to be aware of approval limits associated with the membership delegation approving the expenditure. It also accomodates obtaining additional approval in case of over-the-limit and other requests deferred to Control Points. The additional approver has the option of approving subsequent annual renewals.
The Payment Request for Memberships Form can be filled in on the computer, using Adobe Acrobat Reader 4.0 (or later version), and then printed out and approved. Adobe Acrobat Reader can be obtained free from the Adobe website.
